Community Health & Wellness Partners' Pharmacy is open to Everyone! We're here to help with friendly, personal support, low-cost prescriptions, and even free delivery. We also offer health services like medicine checkups, help with long-term health problems, and screenings to keep you feeling your best.

School-Based Healthcare
At Community Health & Wellness Partners, we believe school-based health centers are a big help for families. They make it easy for students to get great care - like checkups and behavioral health support - right at school. With a parent's okay, students can get the help they need without missing much class. It also helps teachers, keeps care going over time and makes things better for students, families and the whole community.

Senior Care
At Community Health & Wellness Partners, we're here to support your health - especially after age 65. We offer special services like our Annual Care Visit and Chronic Care Management to help you stay healthy and independent.
