At Community Health & Wellness Partners, we want to make sure you can get to your appointments—no matter what!

We work with different transportation services to help our patients get to and from our locations in Bellefontaine, DeGraff, Russells Point, West Liberty, or Urbana. If you need to see a specialist out of town, we can help you set up a ride for that, too. Best of all, this service is free for our patients! Just let us know if you need help with transportation, and we’ll take care of the rest.
