We offer a competitive salary, and great benefits. Medical providers and staff can earn extra based on the quality and productivity of their work. Our sites are recognized by the Ohio Department of Primary Care and the National Health Service Corps (NHSC), which means physicians, nurses and other providers can apply for loan repayment. Our Culture

At Community Health & Wellness Partners, we believe working together makes everything better! Our doctors, nurse practitioners, nurses, therapists, physician assistants, pharmacists, and other staff join forces as one big team to care for people in our community. Community Collaboration

Community Health & Wellness Partners works together with three local hospitals—Mary Rutan Hospital, Memorial Health, and Mercy Health in Urbana—to help people in Champaign and Logan Counties get great care. Our doctors and nurses don’t work inside the hospitals, but they team up with other doctors in the community to make sure patients get the best care possible. We’re the main place that sends patients to these hospitals when they need extra help. We also partner with community groups and social services to support our patients in every way we can.

Community Events

At Community Health & Wellness Partners, we love helping people in our community feel their best! We join in lots of local events, like fairs, festivals, health days, and care days, so everyone can learn about staying healthy and have fun together.

We team up with the Second Harvest Food Bank, and every month their Food Truck stops at our Bellefontaine location to help families get the food they need. We also support sports teams, schools, and health education to help everyone in our area live well.

Every year, we celebrate National Health Center Week with special community events. We offer free services like spinal checks, blood pressure checks, hearing tests, personal hygiene bags, fresh fruits and veggies, and visits from the Second Harvest Food Truck.

Community Health & Wellness Partners is part of a big group of health centers that help over 28 million people across the country! We work on important health challenges like helping veterans, treating opioid problems, and caring for people with long-term health issues. Almost half of all health centers are in small towns and rural areas, just like ours.

As a health center, we work hard to make sure everyone can get the care they need—easily and affordably. We help remove anything that might make it hard for our patients to get care. 

 

We also care about our team! We want our doctors, nurses, and staff to have a healthy workplace and a good balance between work and home life. Our primary care providers are at the center of each patient’s care, and they have lots of support from other team members and outside resources. Providers can even take calls from home, and everyone—medical, behavioral health, and pharmacy staff—can choose to work five 8-hour days or four 10-hour days each week. At CHWP, we always put the families of our employees first!
