Primary Care

Primary care is the first place you go when you need help with your health. It's where you see a doctor, nurse, or other caring professional for things like feeling sick, getting a check-up, or learning how to stay healthy. Primary care providers help with everyday problems, regular visits, and keeping you healthy as you grow up. If you have a health issue that lasts a long time, they help you manage it. And if you ever need special care, they'll help you find the right expert.

 

At Community Health & Wellness Partners, primary care means we care about all of you - not just your symptoms. Our team works together to make sure you get the help you need, all in one place. This team includes doctors, nurse practitioners, nurses, therapists, pharmacists, and more. We want to get to know you and support your health in every way - body, mind, and feelings.
