This year, we’re proud to share that CHWP has received multiple Community Health Quality Recognition (CHQR) Badges from the Health Resources and Services Administration (HRSA).

These national honors celebrate health centers that go above and beyond in delivering accessible, equitable, and effective care. Badges are awarded annually, based on performance data submitted through the Uniform Data System (UDS), and they validate our commitment to excellence while recognizing the dedication of our entire care team.
