Our School-Based Health Centers
Logan County
Logan County offers the best of both worlds. Residents can enjoy life in a warm, rural community surrounded by rolling cornfields while benefiting from the amenities of bigger cities like Columbus and Dayton, located just an hour away. Bellefontaine, the Logan County seat, is a beautiful small community experiencing a youthful downtown revitalization. The city, surrounded by several large companies, including those on the Fortune 500 list, focuses on economic development and providing a desirable place for residents to live, work, and raise a family. It boasts 159 acres of parkland, and the Bellefontaine Joint Recreation District offers a variety of activities, including youth sports, for young people and adults.
Community Health & Wellness Partners supports school-based health centers in 3 of the 4 schools. Rural located, these schools serve students with a focus on providing healthy opportunities for learning. Benjamin Logan is a sizable district that borders the Indian Lake, West Liberty, Triad, and Bellefontaine school districts. The core of the community, Indian Lake is very committed to helping and serving when needed – such as noticed with the tornado whipped through their school district in March 2024. Riverside School District is smaller in size, but mighty in education and care for their students.

Champaign County
Champaign County, located directly south of Logan County, offers rural charm and community spirit. The county seat, Urbana, is the largest city in Champaign County, and has been recognized as one of Ohio's Best Hometowns by Ohio Magazine. The city was honored for its vibrant community spirit, rich culture, excellent education, and strong business environment. Downtown Urbana's redevelopment has made it a regional destination for those seeking fresh flavors and interesting finds. Kiser Lake State Park is the county’s largest body of water and offers sail boating and trails for the horseback rider.
Community Health & Wellness Partners supports school-based health centers in 3 of the 4 schools in Champaign County, Graham, West Liberty-Salem and Triad. All schools are located in rural areas of the county and serve many small villages. Graham School District is one of the largest in the State of Ohio, while Triad is a district that includes three counties – Champaign, Logan and Union, yet is one of the smaller rural schools in the State. West Liberty-Salem is innovative in their approach to healthy children as they were the first to partner with CHWP. These school districts are forward thinking and focused on the whole child for sure.
